If you are a new patient with us you must complete the New Patient Intake form before your appointment.

We will send you a digital from form to fill out before your visit, or you can fill your paperwork out on site 15 minutes before your appointment begins.

A no-show or late cancellation fee may be charged if 24 hours isn’t given. Please allow us time to fill a spot for those who may need it.

No problem, we offer affordable cash rates for all of our services. During your initial visit we can discuss options to help keep your care affordable. HSA and FSA cards can also be used for your care here.

Yes! We joined South Pointe Healthcare at 150 Old Laramie Trail suite 120 Lafayette, CO 80026.

We are now partners with primary care, acupuncture, among other therapies, in addition what we have always offered, including chiropractic and massage therapy.
